Review of Services for People with a Neuromuscular Condition

 

Yorkshire and the Humber SCG has been working closely with the Muscular Dystrophy Campaign to review services provided across Yorkshire and the Humber for people with a neuromuscular condition. As part of this review we want to know what people with neuromuscular conditions, their families and their carers think about the services provided and how they can be improved.

 

This page contains a link to a questionnaire. We are inviting people with neuromuscular conditions and who get their treatment in Yorkshire and the Humber to complete the questionnaire and send it back to us. The questionnaire can be downloaded here and returned either by post or by e-mail. It is completely confidential and you do not need to give us any personal details. If you have access to one of the regional care advisors you may already have received a questionnaire. If you have difficulty in downloading from this page, the regional care advisors will be happy to get a copy to you.
